Hi Kader,
Employee contribution is normally measured in terms of Key result Areas (KRAs). You effort to formulate this draft is indeed laudable. However, I would always suggest an HR personnel to map an employees contribution to the aspirations/goals of your organization. You may consult with your management to derive that or may refer your company score card which may say about the finance,customer (If yours is a service company) ,Operations/process, leraning and growth, Org Development Activities,etc. Please evaluate if you find this insight meanigful. I may not be able to share any template with you at this point in time...:p

Personally, I found it so complicated. Because you didn't describe your company in detail, I am wondering if your middle managers could apply that form.
In addition, criteria for appraisal also depend on the appraisal term. For short term, you need something shorter and more focused on results achieved. I also suppose that you should develop a guideline for appraisers. I am sure that they will get difficulties in making decision that their staff should be rated 3 or 4...You can name several critical behaviors for each level of each competency.

I create this form to follow two different organization’s form. Our company is service oriented company. We have some project. Here I also follow 360 degree performance appraisal system. I know it’s very lengthy but we need all parts of this appraisal. Pls tell me how can I short this appraisal but nothing remove all information.
