Difficult to answer a generic question except by a generic answer.
It really depends on the process-- break it down to the conventional "Men, Materials ,Machine and Methods ..." Pl do also look at the Time and the eco aspects . Ask "Why" five times. Draw an Ishikawa and treat each result as a cause and probe the root cause. It could be the skills of individuals, the efficacy of machines, the communication process.. Find out the weakest link /constraint and address it with all sincerity. Is the cause ego related or network related? Identify the bottle necks(constraints) and debottle neck the process.
Well. If more specifics are known about the process one could perhaps throw more light on the shortcomings.
Best wishes.

First, actually walk the processes! You’ll be surprised at just what a fresh pair of eyes will see. Map the processes, then seek out the 7 wastes.

Dear Mr.Pradeep!
First of all explain your objectives of your process going on to all your staff members.
Then make them to understand conceptually and let them visualise their task. Then explain them about their importance and motivate them through the performance appraisal (either negative or positive).
Then you can see the qualitative and quantitative improvement in your process.
I am wishing you all the success to have the best improvement in your process.
With warm regards
