Always relieving letter heading is : "TO WHOMSOEVER CONCERN " and it should be on the company's letter head.
Then comes the name of the candidate and it goes like this:
Mr ............ who was working with us in the capacity .......(designation) during the period from ...... to .......has been relieved on his resignation, w.e.f. the date of .........

Before relieving an employee, u must make sure that his resignation is accepted in writing. Else, there are all chances of an employee going back and withdrawing his resignation. The same can be a part of the relieving letter also.
